Carey Pensions UK LLP and Carey Pension Trustees UK Ltd have not provided any advice and are not responsible for the suitability or appropriateness of your decision to establish a SIPP.

This application should be used if you are a client establishing a SIPP with advice from a regulated Professional Financial Adviser.

This application should be completed by a parent or guardian if the SIPP is for a child under 16 years of age.

RAS (Relief at Source) Declaration

In order for Carey Pensions UK LLP and Carey Pension Trustees UK Ltd to deal with the tax relief on contributions to your SIPP on your behalf and to deal with Her Majesty’s Revenue and Customs (HMRC) regarding the same, you are required to confirm that the details shown in the application are accurate in all respects and the following declarations are true and made to the best of your knowledge and belief. We have attached a copy of the details you have inputted for your review.

Should there be any changes and where an event occurs which means that any of these details and declarations are no longer accurate, you declare that you will notify Carey Pensions UK LLP as soon as possible and no later than:

(a) by 5th April in the year of assessment in which the event referred to above and in sub-paragraph 2b occurred
(b) within 30 days of the occurrence of that event / the change in your details

If we do not hear further from you in the next 30 days, the declarations you have made in your SIPP application shall take effect from the date of this email.
